An Adam’s apple is a masculine feature; consequently, its reduction is one of the first surgical procedures sought to transition from male to female (MTF). The procedure is a small but important step in the transgender change process.

About the Procedure

A laryngeal shave is an outpatient procedure using local or general anesthesia and can be done through a direct skin incision over the trachea or under the chin in about an hour.

Since the vocal cords attach approximately halfway down the front portion of the laryngeal cartilage, it is only the upper one half of the cartilage that can be reduced. Fortunately, most of the Adam’s apple is in the upper half.

Because of the potential risk of damaging the vocal cords and changing one’s voice, the surgery is performed under local anesthesia. By doing so, Dr. Silver can hear the patient’s voice or use a laryngoscope to locate the vocal cords. The size of the larynx influences the depth of the voice. The larger the laryngeal prominence, the deeper the toned voice. Once the larynx has been trimmed to the desired level, Dr. Silver will close the wound with tiny sutures to minimize scarring. A bandage will be placed over the site to prevent infection or other complications.

Aftercare and Recovery

Recovering from a laryngeal shave procedure is relatively simple and the incision requires little care. There may be swelling and firmness around the wound for a few weeks; however, patients have reported only mild discomfort allowing them to run, workout, and do any non-contact sporting activity. After a few months, the incision will heal and become barely noticeable. The end result is a much smoother and more feminine neckline.
